Assured Healthcare has a two hour minimum of service per week.
A client referral may be taken at any time by Assured Healthcare. An RN Supervisor will open each case after the initial referral. A complete assessment will be done on each client and a care plan will be developed from the assessment, which is then sent to the client’s physician. We encourage client and family participation in the care plan so that all the client needs and expectations are met. Assured Healthcare’s employees will follow the care plan for each client.
Once a client has requested service from Assured Healthcare, we respond to that request with the information of who will be coming to their home and the schedule for what the client has requested. If a client requests services for more than four hours a day, a monthly schedule is sent to the home so that it may be posted and the client knows who is coming to care for them on a daily basis.
Assured Healthcare strongly believes in continuity of care for the patient. All efforts will be made to keep the same employees on the case for the duration of the case. This allows the client to know their caregivers and feel that they don’t have a different person in their home on a daily basis.
Client satisfaction surveys will be sent to each client to evaluate Assured Healthcare and its employees.
