Referral Information for New Clients

Assured Healthcare will take a client referral at any time. Registered Nurses will do a physical assessment on each client, and from that assessment, a care plan will be developed and followed by Assured Healthcare employees. We encourage client and family participation in developing the care plan to ensure the client's needs and expectations are fully met.

  • Assured Healthcare strongly believes in the continuity of care for the client.
  • Assured Healthcare has a two-hour minimum for caregiving services per shift
  • Assured Healthcare has a four-hour minimum for nursing services per shift
  • Professional references provided upon request.

New Client Referrals

A client referral may be taken at any time by Assured Healthcare; we answer the phone 24 hours a day, seven days a week. An RN supervisor will open each case after the initial referral. The RN supervisor will do a complete assessment on each client and develop a care plan, which is then sent to the client's physician. We encourage client and family participation in developing the care plan so that all the client's needs and expectations are met.
